April 2009: Backup tapes containing patient information on patients of Peninsula Orthopaedic Associates were stolen while in transit, the practice notified approximately 100,000 patients. The tapes have not been recovered.

June 2008: Backup tapes stolen from University of Utah Hospital
The University of Utah Hospitals & Clinics announced today the recent theft of billing records. A metal box containing backup tapes, which contained billing records for approximately 2.2 million patients and guarantors, was stolen on Monday, June 2, from order lipitor online a car belonging to a driver who order lipitor online worked for an independent storage company contracted by the health-care system.
